To schedule an appointment, please call our office at (812) 372-8293.

A physician is on call after business hours. For emergencies, call the hospital operator at (812) 379-4441 and they will contact the doctor.

If you are having chest pain, shortness of breath, bleeding or a life-threatening emergency go the to Emergency Room or call 911.

If you are experiencing signs of an illness, an appointment is necessary. When scheduling an appointment, please explain the reason for your visit. This allows us to schedule the appropriate time needed to address your healthcare concerns. For your first visit, please arrive 15 minutes prior to your appointment time so that we may obtain the information required for establishing your care.

• Each visit bring your insurance card with you as well as a list of your current medications.
• Please inform the front desk staff of any change in name, address, telephone number, employment or insurance.
• Always inform your physician of any changes in medical history and bring a list of medications.

If you have an urgent need to be seen, please call as soon as possible, and we will try to accommodate you. We do our best to stay on schedule, but emergencies may sometimes cause unexpected delays. If you are running late, please phone before you come as you may be asked to reschedule. If you can't keep an appointment, we request a 24-hour notice. If a patient misses four appointments within a six-month period without providing prior notice, they will be discharged from the practice.
